The practice is keen to provide prospective access to those patients who wish to have this access. However, rather than providing automatic unchecked access, we are opting for a more robust and controlled process where patients must have their identity verified, and sign a consent form before our GP's are able to run clinical assurance checks before granting access. This is to ensure that all patients requesting access to their patient records understand all potential risks, and also for us to be able to maintain our duties and legal responsibilities as the data controllers of patient records. Please download and print the application form then visit the surgery in person and hand into reception. Please show the reception team your forms of identification. We are not sure how many people will ask for access all at once so there may be a wait, but we will do our best to get you online access as soon as we can.
