1.  If you are asking for proof of vaccine and had both in the UK under the NHS. 

You can access your vaccination status in either of two ways.

Through the NHS app.  Proof of your Covid 19 vaccination status will be shown within the NHS app.  We recommend that you register with the app before booking international travel

By calling 119. If you do not have a smart phone and know the country you are travelling to requires Covid 19 vaccination status you can call the NHS helpline on 119 and ask for a letter showing your vaccinations to be posted to you.  This must be at least 5 working days after your second vaccination.  Allow 5 working days for the letter to get to you.

2.  If you want proof of vaccination but had either dose outside of the UK.

Send your surgery proof of vaccination.  This must include type of vaccine, date given and batch number.  The surgery can add this to your medical record, and it will be on your file as an immunisation, but it will not appear on the NHS app under covid vaccination status.  It will only appear in your immunisation history.  There is currently not a central mechanism for capturing details of vaccinations administered outside of England.  Only vaccines delivered in the UK can be accepted as evidence for UK/Covid certificate/the NHS Covid Pass.   A technical solution to support the recording of vaccinations given outside of England is in development.

3.  If you had your vaccine at another clinic and it is not showing on your medical records/NHS app a week later.

Please send in your proof of vaccination with details of where it was given and we will follow this up with the Oxfordshire Clinical Commissioning Group.

4.  If you want an early 2nd vaccination for travel.

Unfortunately, a 2nd vaccination cannot be given early for travel reasons.  Current guidance is that there must be 8 weeks (56 days) between vaccinations.

If you need patient access linkage contact the surgery and we can supply this.  Please contact the NHS App help desk for any other support ( you can do this from the NHS APP itself)

We cannot provide letters showing your Covid 19 vaccination status, please do not contact us about this.
