Patient responsibilities

Cancelling appointments

Please phone in good time to alert us of any appointments you cannot make - this allows us to offer your appointed slot to someone else.

Confidentiality

We respect your right to privacy, and keep all your health information confidential and secure. However, the Department of Health may ask us to disclose this information for national surveys and medical studies - you must inform the practice manager in writing if you do not wish to have your details used in this way.

Access to medical records

Requests for access to medical records should be sent in writing to the practice manager. Each request will then be considered under the guidelines of the Data Protection Act 1998.

Change of address

It is important that we have up-to-date records about your personal information, including your name, address and contact details. Please inform reception or fill out our contact details form if any of your information has changed.

Zero tolerance

The practice will not tolerate abuse to staff and considers aggressive behaviour to be any personal, abusive and/or aggressive comments, cursing and/or swearing, physical contact and/or aggressive gestures. All such incidents will be dealt with according to 28 Beaumont Street's Practice Policy, which gives us the right to:

  • Remove any patient from the practice list who is aggressive or abusive towards a doctor, member of staff, other patient, or who damages property; and
  • Report all instances of actual physical abuse on any doctor or member of staff (by a patient or their relatives) to the police as an assault.
