Registering with us

All you need to know about choosing 28 Beaumont Street

NHS patients

If you wish to register with us, simply pop into our surgery during opening hours and pick up an NHS Registration Form and confidential health questionnaire.

Once you have had a health check with one of our nurses / healthcare assistants, we will register you and give you the details necessary to use our online facilities, including appointment booking and repeat prescriptions.

New registrations who require repeat prescriptions will need to meet with a doctor to arrange this. Please speak to a receptionist when you register, or call 01865 311811 to book an appointment.

Private patients

The registration process for Private Patients differs from NHS Patients - please contact us for more information or to arrange registration.

Oxford University students

All new Hertford College and New College students are registered at the start of the Michaelmas term. If you missed the registration for any reason, please contact us to arrange an appointment as soon as possible.

Once you register with us, your medical records will be transferred to 28 Beaumont Street for the duration of your studies. You do not have to register with us, but we are familiar with the way each College works and run surgeries in College.

If you choose not to register with us, or transfer to another Practice, please let the College know the name of your Doctor in case of emergencies.
