Freedom of information

You may request  the information we hold about you in line with the Freedom of Information Act. We will not disclose your information to anyone outside the NHS without your prior written permission, except in exceptional circumstances  when the law requires the information to be disclosed.

To see your medical records, please apply in writing. We will arrange a time for you to come in  to view your records with a member of staff, so that any abbreviations and medical terms can be explained. Alternatively, if you would like us to give you a copy of your medical records, please apply in writing (a  charge may apply for this service).
