What to do if you have a complaint Freedom of Information Medical Students in the Practice Changing your address and contact details Confidentiality Adults (over 16 years)All medical information coming through the practice is treated as confidential and our staff are all bound by strict confidentiality rules. For this reason we are only able to give medical details to the
patient concerned. We cannot give information to relatives unless we have permission to do so (in writing) from the patient concerned. Only in medical emergencies, or very rare instances when the law
requires us to do so, would we reveal medical information without a patient's consent. Teenagers (under 16 years) Teenagers under 16 are entitled to confidentiality, and can seek medical
advice (e.g. about contraception) without worrying that their parents will be informed. We generally encourage teenagers to discuss their health problems with their parents but will respect their right to privacy. Once a teenager reaches an age where they are competent to make their own decisions about their health, we are required to consider both the wishes of the teenager concerned and our duty to protect them from
harm. Our most important duty is to protect children from harm and we will always aim to act in the best interests of every child. Younger children An adult with parental responsibility
is generally entitled to medical information about their child. Only rarely, when it is considered that harm to the child might result, would a parent be prevented from access to their child's health information.
What to do if you have a complaint We operate a
practice complaints procedure as part of the NHS complaints system. Our aim is to give you the highest possible standard of service and to deal swiftly with any problems that you may encounter.If you have
a complaint the first step is to inform us by contacting our acting Practice Manager who will give you further information about what to do next. If you prefer to discuss your complaint with someone outside
the practice, you can contact Mrs Sue Smith on 01865 762535. Also, you can contact
Oxfordshire
PALS (Patient Advice and Liaison Services). This is part of the NHS
plan to improve patient experience. Freedom of Information You may request
the information we hold about you in line with the Freedom of Information Act. We will not disclose your information to anyone outside the NHS without your prior written permission, except in exceptional circumstances
when the law requires the information to be disclosed.

to view your records with a member of staff, so that any abbreviations and medical terms can be explained. Alternatively, if you would like us to give you a copy of your medical records, please apply in writing (a
charge may apply for this service). Medical Students From time to time, First and Fifth year medical students from Oxford University spend time in the Practice for training. You may be asked if one of them can sit in on your
consultation or talk to you before you see the doctor. This is entirely voluntary; if for any reason you are not comfortable for a student to be present, please inform the receptionist or tell your doctor. You are also
free to change your mind at any time during a consultation. Change of Address Please notify us immediately of any change of address or telephone number by ringing us on 01865 311811 or request changes to your details online. This prevents any problems when sending you
